Gyermekorvosként, védőnőként vagy szülésznőként naponta tapasztalhatja, hogy nem csak üres szólam, hanem tény: nincs hatékonyabb, olcsóbb, átfogóbb hatású prevenció a szoptatásnál, amely az anya, a gyermek, a család és a társadalom számára is létfontosságú biológiai és lelki szempontból egyaránt. Sikerében azonban döntő jelentőségű az a támogatás, segítség, amit az édesanya olyankor kap, amikor elakad, nehézségekbe ütközik - manapság ez sajnos nem is olyan ritka. A nehézségek olykor épp a szülőszobán, a gyermekágyas osztályon kezdődnek. A szülésznő, a kórházi védőnő vagy a neonatológus, aki ebben az érzékeny, jelentős időszakban kerül kapcsolatba az édesanyákkal, biztos pont lehet az egymásnak sokszor ellentmondó, zárporozó tanácsok sűrűjében.

Annyi bizonyíték gyűlt össze az igény szerinti szoptatás mellett, hogy nehéz kétségbe vonni létjogosultságát. Az is kiderült, hogy az "időre szoptatás" sokszor nem működik, sőt, káros következményei is vannak. Miért nehéz mégis igény szerint szoptatni?

Prepregnancy Weight, Weight Gain during Pregnancy, and Exclusive Breastfeeding in the First Month of Life in Rio de Janeiro, Brazil

To examine the relationship between maternal prepregnancy weight, gestational weight gain and early introduction of non-breast milk foods and fluids (EINB) in the first month of life, we investigated 592 adult women and their newborns, admitted at health care units in Rio de Janeiro, Brazil. EINB was defined as the introduction of water, tea, juice, other types of milk or food during the first month postpartum. Logistic regression models were used for the analyses. Prepregnant obese women had increased odds of EINB as compared to those with normal weight (odds ratio [OR] = 2.14, 95% confidence interval [CI] = 1.23-3.71). Overweight and obese women who exceeded the recommended gestational weight gain had significantly greater odds of EINB (OR = 2.29, 95% CI = 1.16-4.51, and OR = 3.33, 95% CI = 1.49-7.47, respectively), compared with those with normal weight and adequate gestational weight gain. These findings highlight the importance of providing overweight/obese women with proper lactation guidance in the early postpartum to support for exclusive breastfeeding practices.

Provision of Support Strategies and Services: Results from an Internet-Based Survey of Community-Based Breastfeeding Counselors

This study examined the determinants of support strategies and services provided by community-based breastfeeding counselors (CBBCs) and compared differences in extent of support provided by paid and volunteer counselors. Participants (N = 847) in this internet-based survey were mostly White/Caucasian (74.9%), college-educated (59.0%), and paid CBBCs (63.8%). The majority (75.9%) of volunteer CBBCs compared with their paid full-time (52.1%) and paid part-time (47.4%) counterparts had completed college. Being a full-time paid compared with volunteer/unpaid CBBC was associated with face-to-face counseling (OR = 3.69; 95% CI: 1.93, 7.06), use of client-centered counseling skills (OR = 6.23; 95% CI: 3.40, 11.45), making referrals to social service agencies (OR = 13.18; 95% CI: 6.86, 25.32), and helping position baby (OR = 3.77; 95% CI: 1.64, 8.69). Because of the disparities in CBBC usage of breastfeeding support strategies and continuing education between paid and volunteer CBBCs, there is a need to examine differences in training curricula and determine the facilitators and barriers of continuing education.

Development and Piloting of an Instrument That Measures Company Support for Breastfeeding

Background: Maternal employment has been cited as a barrier to continued breastfeeding, yet there have been few studies identifying company breastfeeding support.

Objectives: The study objectives were to develop and pilot an instrument that measures formal breastfeeding support in companies.

Methods: A survey of company breastfeeding supports was developed, based on the literature and expert review, and the survey piloted with Michigan-based companies.

Results: Completion of the surveys and open-ended comments indicated survey items were generally well understood. In the study sample (N = 151), significantly more support was offered in companies having more employees as compared to fewer, and in companies from the health care versus all other sectors (P < .01). More support was also found in companies reporting requests for support, upper management combining breastfeeding and work, and a population that they felt likely to require breastfeeding support (P < .01). Few companies (3%) had written policies on breastfeeding or pumping at work. However, the majority of companies allow women to pump milk at the worksite (94%), and provide time (73%) and nonrestroom space to pump (78%). Fewer companies allow breastfeeding at the company (22%) or designate a room solely for breastfeeding or pumping (32%).

Conclusions: The survey instrument is appropriate to determine breastfeeding supports in companies. In Michigan, larger companies and those in the health care sector provide more supports, most companies provide some type of space and time to pump, but other supports are limited.

Breastfeeding Duration in Relation to Child Care Arrangement and Participation in the Special Supplemental Nutrition Program for Women, Infants, and Children

This study was conducted to examine associations among breastfeeding duration, child care arrangement, and participation in the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC). The authors analyzed the nationally representative sample of infants born in 2001 who were enrolled in the Early Childhood Longitudinal Study–Birth Cohort (9 months). They estimated the risks of short breastfeeding duration (< 6 months of age) among infants according to the child care arrangement and WIC participation (n = 7515). Approximately 50% of US infants were enrolled in WIC, and 47% received child care on a regular basis. Both WIC participation and child care use were independently associated with short breastfeeding duration. Compared to those under parental care, WIC participants under relative care were more likely to discontinue breastfeeding before the age of 6 months. A breastfeeding support program targeted toward relative care users may be useful for WIC participants.

Sharing of Bacterial Strains Between Breast Milk and Infant Feces

In previous years, it has been shown that human milk is a potential source of bacteria for the infant gut. The results of this work confirm the presence of the same specific bacterial strains of Bifidobacterium, Lactobacillus, and Staphylococcus in breast milk and infant fecal samples. The identity of bacteria isolated from breast milk and infant feces from 20 mother-infant pairs was investigated at the strain level. DNA from Staphylococcus, Lactobacillus, and Bifidobacterium was detected by qRTi-PCR in nearly all samples analyzed. These samples were cultured on different agar media. One colony representative of each morphology was selected and identified at the species level combining classical tests and molecular techniques (PCR, RAPD, PFGE, and/or MLST genotyping). Breast milk and infant feces from 19 mother-infant pairs shared different Staphylococcus, Lactobacillus, and/or Bifidobacterium species and strains. Significantly, 2 mother-infant pairs shared 4 bacterial strains although most pairs shared 2. These results confirm that breast milk and infant feces from mother-infant pairs share the same strain(s), indicating that breastfeeding could contribute to the bacterial transfer from the mother to the infant and, therefore, to the infant gut colonization.

Determinants of Exclusive Breastfeeding in a Cohort of Primiparous Periurban Peruvian Mothers

The authors aimed to identify factors associated with exclusive breastfeeding (EBF) among 117 Peruvian mothers planning to breastfeed exclusively. Data were collected on days 0 and 3, and months 1, 3, and 6. Exclusive breastfeeding status was evaluated with a 24-hour recall of infant diet. Exclusive breastfeeding rates were 74%, 72%, and 35% at 1, 3, and 6 months, respectively. At 3 months, lower maternal education, greater breastfeeding frequency (day 3), greater breast pain (day 3), and depot medroxyprogesterone acetate use (3 months) were associated with EBF, after adjusting for EBF intentions. At 6 months, greater infant birth weight and mother–not employed were associated with EBF, after controlling for EBF intentions. More educated and working mothers, and infants with lower birth weight should be targeted in interventions to promote EBF in urban Peru. Research is also warranted to explore the factors linking depot medroxyprogesterone acetate use and breast pain with EBF duration.
